CAPH/SNI is pleased to invite you to our Improving Patient Inreach & Outreach meeting, Thursday, August 15, 8:30am-3:15pm at the Oakland Airport Hilton. At this meeting, members will understand how successful outreach for assigned-not-yet-seen patients supports improved care and performance in PRIME and QIP with attendees sharing impactful practices for increasing the accuracy of patient data and optimizing outreach for assigned-not-yet-seen patients, learning how to tailor outreach based on patient demographics, and taking away implementation lessons around outreach practices like centralized telephonic outreach and patient portals.

Additionally, participants will practice incorporating the patient voice and preference while designing and improving processes.

Attendees will include PRIME/QIP leads, data integration leads and ambulatory care operational leaders. A draft agenda is attached. Once you register, short pre-work will be assigned in July.

Please register as soon as possible to ensure we get an accurate head count and pre-registration information.  SNI can reimburse travel expenses for up to 4 people from each system; our travel policy is attached.
