Active Care Teams (ACT): Embracing Daily Team Huddles
Being prepared for a busy clinic session is an important part of any high performing practice. Implementing daily team huddles is an effective strategy to prepare clinic staff for the day and to avoid missed opportunities with patients. While seemingly simple, team huddles can be a powerful tool to enhance care coordination, improve patient and staff experience, and maximize efficiency.
This ten minute video features a team from Alameda County Medical Center’s Winton Wellness Center demonstrating techniques for an effective daily team huddle. The video shows the process by which this provider and medical assistant team spend several minutes each day familiarizing themselves with the patients coming in, identifying specific care activities needed for each patient, and delegating tasks to members of the care team.
This video can serve as a useful training resource for clinics planning to implement daily team huddles. For those clinics already conducting huddles, the video can be included as part of the new employee orientation and training.
